Payment for Services

Please plan to take care of payments or outstanding balances at the time of service.  We accept debit cards, Visa and Mastercard.

If we are not participating providers with your insurance plan, or you provider does not cover services provided, you will be responsible for any charges incurred. We will provide you a receipt for your insurance company.

We realize that emergencies do arise that may affect timely payment of your account. If such extreme cases do occur, please contact a patient account representative at 208.535.4565.

Any check returned from the bank will result in an additional $20.00 charge.

Always notify our office of any change in name, address, phone or insurance information.
